You need to do 3 things to add a sales form to a sales page.

First, Build The Form In Your Shopping Area.

Click on this area, and you’ll see a drop-down menu. From here, you can select the action you want the user to perform after submitting the form.

You can choose to “Submit for review” or “Display a different form.” You’ll use the second option, as the first one will send the user straight to your shopping cart.

Once you’ve connected the form to a suitable action, you can test the whole thing by clicking on the “Submit for review” button.
